There's many reasons why the turtles could be killed in any scenario.
Aside from obviously "Same as it Never Was"
I know Don was killed in the Image comics by Deathwatch
and i know it's different but i remember Leo died in a dream during a story arc where he was blind... i think it was called "blind faith"
Are there any other instances where the turtles have died?
and in any scenario, who do you think would really die first?
i know everyone doesn't like to talk about our favorite guys dying, lets humor ourselves shell we? <-lol see what i did there ?
it's really hard to say for me.. I'm a bit undecided what my opinion is, but I'll just go ahead with my favorite turtle Leo and say his bravery would make him sacrifice himself for his brothers:tcry:

I voted for Leo as I feel he would die to protect his brothers. And the arc you're referring to is called "Blind Sight."

It wasn't a dream in Blind Sight, Leo was well awake when he was standing on that tower. It was his thoughts wandering what would happen if he fell down, or just an alternate side story on what would have happened of Leo fell down.
The dream sequence was when Splinter gave Leo the Infi-tea.

Don seems to live the longest in Mirage as we can see him alone, very old and blind. reliving old memories with holograms of his brother father and friends.

I could easily see Raph as the first to go, simply because he has that attitude of not caring what happens to himself as long as his brothers are safe. I'd almost call it a death-wish, but I'm not sure. Leo would try his best to save them ALL, especially knowing that sacrificing himself would also mean that they would be more vulnerable in the future. He thinks ahead for the long-haul. Raph- not so much....

Well, Leonardo certainly has the "protect until my last breath" attitude, so I voted for him in most scenarios but truth be told, I could see ANY of the turtles going first if the situation called for them to protect each other.
Leo usually gets the credit for being the most protective but Raph is not far behind and honestly, I don't think Don and Mike get the credit they deserve. They can be pretty loyal and devoted as well.
For me, it would all depend on the situation: Leo would die to protect any of his brothers, Raph the same, Don would be trying to keep them all together and Mike would probably think the others were too important to the mission to lose....

I always saw the turtles dying at old age, something that seems most of the Mirage Turtles probably did.

I always feel it will be Mikey but only because he acts like the heart of the group and his death would be the biggest factor of them breaking apart or at least splinting up on such bad terms.

I disagree that Mirage ever implies Mike dies first. In fact, we don't really know what happens to him, as he's the only Turtle we never see in his old age.
I think there is a pretty heavy implication that either Don or Leo die last, though ("Loops," "Swan Song" and the 2007 Tales frontispieces show them both living alone and are filled with numerous allusions to the absence of the other Turtles, but it's not clear which would come first or when either of them bite it). Both definitely seem on their way toward going of old age.
Raph, as well, I think, is heavily implied to die first in Mirage. Not only due to his lifestyle, but also due to the fact that he appears visibly younger than the other Turtles in Donatello's "reunion" simulation (2007 Tales frontispieces). While it could be interpreted as just being the last way Donatello saw him, I think the easier way to read it is that that he was killed much earlier than the rest.
Leo will always die first if they are all together. He would sacrifice himself to save the others.
Which doesn't happen in at least Mirage, as they all split up around their middle age.
They seem to stay together in both the Fred Wolf cartoon and Adventures, though. So I could see that.
I'd almost call it a death-wish, but I'm not sure.
Not sure about other universes, but Mirage Raph definitely has a death wish. Dude is f*cked up.

I could see Leo being killed in defense or protection of the others, but honestly I more practically see Raph getting killed for once again rushing into a bad situation without thinking it through, being taken by surprise and killed. Probably not being out-fought, more likely taking on one too many bad guys and one gets the drop on him from behind, possibly shooting him in the back of the head before he knows it's coming. Not the most heroic death, but to me it's easier to see than someone actually being able to get over on him legitimately. He thinks he's won the fight, feels cold steel against the back of his head, hears a click and has less than one second to realize - too late - that Splinter and Leonardo were always right to warn him that his impetuousness would one day get him killed...
Again, maybe not the most heroic of deaths but that's kind of how I see it in my mind's eye. Guy always rushes into a fight without thinking, only stands to reason that one day his luck would run out if he was all by himself.

Mikey dying first would definitely rob the Turtles of their most "innocent" member and probably blighten their view on the world and on themes of togetherness. I don't think he dies first though, I often like to think he's the one out of all four that actually manages to settle down with someone and just goes off the grid.

I kinda see that, too. Of the four, Mikey seems the most likely to abandon "the life" and simply try to live a life away from so much conflict and violence. He doesn't seem like he would want to live a warrior's life forever.

I agree with that. I think of all of them Leo and Raph would stick to the lifestyle that they have had while Don and Mike may try to shy away from it and branch out on their own.

In terms of natural death:
1) Leonardo
2) Donatello
3) Raphael
4) Michelangelo
*These are all based upon their age.
In terms of diet/exercise:
1) Michelangelo- too much pizza
2) Donatello- too many video games, but at least his diet is decent
3) Raphael- Apt to injure carelessly, plus watches too much TV
4) Leonardo- Best diet/exercise; well-rounded.
If they faced the Shredder alone:
1) Donatello: No gadget trick along with a piece of wood against a sword doesn't bode well for him to last long.
2) Michelangelo: A bit too slow
3) Raphael: Fast and the sai could throw Shredder off-guard, but lack of sharp weapon will eventually take care of him.
4) Leonardo: With the katana, the best shot of defeating Shredder.
As far as I know, the Turtles have never died outside of the videogames or a dream or the couple rare mentions by the OP.

I think Raph would be the first to go in most storylines. His hot temper and headstrong attitude would eventually be the downfall of him. He was beaten t near death in the first film, and he's also the one turtle ho who try fighting Shredder alone (besides Leo).

I thought the implication in the Mirage universe was not so much that they died but that they went their separate ways, with Raphael becoming more and more primitive/animalistic as time went by, and Donatello becoming more and more advanced due to his inventive nature. In Mirage, it seems they all die but they're out of touch with one another and don't really know who goes first (at least that's what I've always thought).
In my fan-fiction, I think Leo dies first, followed by Don, and Mike dies in Raph's arms and Raph is left all by himself. But they always show up in one another's lives again, due to the time-slip device and Renet's meddling.

Ditto to that. They are much less naturally inclined to spend their lives fighting, and neither strikes me as the "true warrior" type like Raph or Leo. They would be more likely to die peacefully of old age- after leaving behind the life of fighting for good.
And I totally see Raph going first due to his hot-headed nature, like others have said. He'd be the one to go off half-cocked and get into a situation he had no way out of.

Isn't there no final resolution on what exactly happens to Mirage Michelangelo anyways? Unlike compared to the other three Mirage brothers? (I guess its assumed that Michelangelo is lost in space after that whole Azokk/Triceraton ordeal, and is the first one considered "dead". ...Isn't there a scene were Raphael laments about Michelangelo's whereabouts in a flooded NYC with a robot version of April?)

I can agree with this. With Michelangelo eternally missing, it could be the catalyst as to why the remaining three Mirage Turtles have split apart in their old age. Otherwise why would these 3 brothers want to split apart when the comic and Turtles have constantly been wistful over their solitary uniqueness? ...Unless something severely tragic happened? Which I assumed was Michelangelo missing and since he was a peacemaker.

I agree with this too. I think if Michelangelo died tragically first, it would send the other three brothers into a far darker and pessimistic path into a whirlpool of depression since these four were already isolated from humanity, and not having Michelangelo's constant optimism and effervescent personality would be like an emotional "nuclear winter" for them. At least with Michelangelo he could understand their unique existential situation and the brutal fights they encountered, but with him gone... It would make it impossible for anyone to "understand" them while being able to show them the "brighter" aspects of life.
I think if there was an opportunity for him to leave the whole ninja fighting life without worrying about being hunted down, or persecuted, he would (as with Donatello).
Anyways... I could easily see Raphael being killed first because of his impetuous behavior.

Raph, he's the one who loses control and doesn't think things through, and he's a thrill seeker and part of him, (Mirage/2K3) has a bit of a self destructive element to him,
Leo and Don would probably be the most cautious and level headed ones, ...

Actually, in most cases aside from the video games... Mikey is the fastest Turtle with Raph being the strongest... for some reason they switched that in the classic Arcade games

I picked Leonardo based on enemy tactics. You always take out the leader. In Baxter's Gambit, despite Xever's feud with Raphael, he chose to throw the bomb(?) at Leonardo at the end of the episode.
