I know a long time ago, when Mikey & Raph faced Green Mantle, that Don & Leo had the "bye week".
Today on the HUB's NT;TNM, there was an episode where Mikey & Venus missed out (go ahead and cheer anti-Venus fans). So that leaves Raph winning the perfect attendance award, I think.

Stories where all four turtles aren't present would be my guess.
I can't think of one off the top of my head. I know in an Archie mini-series, Raphael got separated from the rest of the Turtles and was on a ship with Mondo Gecko. So I guess that counts because the turtles were absent from the rest of the story (an issue or two).

I guess he's looking for stories where one, or more, of the turtles is absent, leaving the others to deal with the situation (often deadly) without them.

Why do you always have to shout?

It's my favorite font & size. I got Mr. Magoo eyes. Shouting is using all capital letters!

I have to admit, it's pretty funny to use this font and size. No, it's actually really annoying.

Hmmmm, besides NM, I'm pretty sure all turtles have been together or atleast seen during an episode.

So, there was never an episode of either the 1987 classic, Next Mutation, 2K3, Fast Forward or Back to The Sewers where Raph was absent. Guess he gets the perfect attendance award.

You forgot about that season 4 episode of the 4Kids series where Leo and Casey teamed up, so none of them have perfect attendance.

Dragon's Brew. It remains the only episode outside of the comics to only feature one Ninja Turtle. There is also no Splinter, Shredder, or April, which means that of the franchise's seven most popular characters (arguably eight of you include Krang), only one is featured! Therefore, Leo and Raph are tied with only one missing appearance. Don and Mikey are tied for last with two missed TV appearances.
But in terms of other media including the comics, it has to be the April O'Neil micro (the original arc). In these three issues, the TMNT never appear at any point live! In the first issue there is a picture of Michelangelo on one wall and at the end of the third issue, there is a picture of the four Turtles and April saying "THE END".

Unless there's a story arc involved, all four Turtles would have to appear in every episode. Even if they were seperated whether individually, in pairs or three out of four, the viewpoints would balance out. Once in a blue moon, total absence would be alright depending.
