He turned himself into a cyborg in the Mirage Volume 2 comics, from what I understand Baxter preformed Nanobot surgery on her.

From his Bio:
A moderately insane scientist. Working with his assistant, a young April O’Neil, Dr. Baxter Stockman designed and built an army of robotic "mousers," which he had allegedly created to effectively rid New York of its unwanted rodent population. He instead intended to utilize his mousers on a grander scale, using them to rob banks and hold the city at ransom. When April discovered this agenda, Baxter feared she would notify the authorities, so he sent his mousers after her. After being rescued by the Turtles, April returned with them to stop Baxter, fighting their way through his mousers to stop his plans short, then leaving him to be arrested (TMNT Vol. 1, #2). Baxter somehow managed to escape prison (that or they simply released him) and infiltrated a DARPA research base in Nevada. Killing the guards, he used what equipment they had to construct a robotic body, then had his machinery surgically transfer his brain into it. Now a cyborg, Baxter took his glasses and made his way to New York, where he sought vengeance against April O’Neil and the Turtles. His plans fell short, however, when Leonardo overloaded him with electricity in a sewer confrontation. Exploding, there was little left of Baxter but his glasses (TMNT Vol. 2, #2 - 9).

Flash forward to Volume 4. It was discoverd that when Baxterbot attacked and stabbed April, he'd injected her with a Nanotech Virus. It laid dormant for a long time, and suddenly started ravaging her body. Donny went to a secret area of his lab, and there Baxter Bot was tacked up to the wall. Donny got what info he needed out of Baxter, and then blew him away with a blaster. The Utroms created Turtle Nanotch robots to send into April to battle the virus. And the turtles were hooked up to pods, so the mini-robo turtles could have their moves and training. During the battle, the communication lines were blown up, and the Turtle Nanos started to loose. The Utrom doctor that was monitoring them, had secretly captured mental patterns of the turtles and uploaded them to the remaining turtle bots. Who took out a Super Nano-Bot that was about to sever April's brain stem. There were only 4 Turtle Nano-Bots left. Each one posessing mini-weapons and the mental pattern of the corresponding turtles. They were kept for study. :)

You left out the best part.....
In Vol. 4 #9 it is revealed that Donatello kept Baxter's robotic form alive for years imprisoned in a sewer hideout. Donatello used to speak with Stockman every now and again about matters of science. After years of leaving Baxter to solitude, Donatello returns to ask Stockman how to stop the nanobots that he injected in April. When Baxter refused to help, Donatello took a Triceraton gun and blew him to bits. Read my signature to find what he said after he did so.

Baxter came up through the sewer hole in volume 2 and poked April right? And that's why she has nanobots in her in volume 4 right?
