Took the liberty of scanning the pages he's on in "Shredder Gets Splintered An Adventure To Color", the Random House coloring book from 1988. This is the only known instance of the use of the name "Scrag". I'd assume they got it from David Wise's scripts, but can't say for sure.

I had the "Splinter's Last Chance" storybook, which told it was Bebop who were mutated back by Shredder's retromutagen ray.
