Is Leo the only Turtle with spots like that? I don't see as many Don and Raph closeups. Mikey obviously has his freckles.

No, Raph has them too. There just harder to see.

Actually, they all have them. I looked at Don's face once, and I thought I saw some spots on him, too as well as on Raph. You just have to look for them. Makes me think that what Don said about them being descended from the diamondback terrapin could be true after all and a bit different than them being descended from a red eared slider.

He's slipping a cockroach into the back of Raph's shell - it happens at the very end of the Cockroach Terminator episode.

It's from cockroach terminator. Raph was bragging about conquering his fear and Mikey tells him he just slipped a bug into his shell.
