annoying/funny errors

I was thinking first just have the thread about artwork, but I figured make it a general question

what errors makes your head turn?

personally I can't stand when the belts are not completed. When they stops right before the shell or are drawn to go under the shell.

funny errors have to be changing masks, voice actor weapons in the early OT
and mikey's return of the stocking bandage in a stock footage
