Duoshade tones were missing in a couple of issues in previous volumes of this collection as well, I'm sure at least one was in the City at War arc or in Shades of Gray (ironic, uh?). I saw this happening in an italian reprint of those same issues years before so the problem is either Mirage sending wrong/incomplete material to Viacom/IDW or IDW using wrong version of those.
All in all I believe this is actually pretty cool. If you have a copy of the original issues it is cool to compare and see how much the tones radically change the way the pages look.
EDIT: Just checked: in Ultimate Collection vol. 4 issues #49, #53 and #54 were missing Duoshade tones. I can't believe that went unnoticed to the guy who put the book together but as far I can remember there never was an official statement from IDW about it.
