So she's got her secret mutant boyfriend, Murakami's is a common date night spot, otherwise Donnie gives a nod to the old days and disguises himself a bit, and in some years, if it lasted, you permanently move to Northampton where you'll be left alone.

Casey would just have to get over it, April's father - assuming he can be found and saved again - already knows Donnie and maybe could come to accept it if it's what his daughter wants, Splinter likewise in reverse, Donnie's brothers will eventually give it a rest, if a real Irma shows back up she may eventually know the Turtles and, while I imagine there would be some disagreeing and debating it she's probably eventually give up and accept an April-Donnie thing... All the important people could deal with it. For the rest of humanity, none of their business if April just doesn't want to discuss her love life.
